An exciting opportunity has arisen to work as part of the Leeds Mental Wellbeing Service as a Primary Care Mental Health Support Worker.

Your role will be to work within Leeds’s locality based Local Care Partnerships, providing mental health support as an integral and critical part of a multi-agency and multi-disciplinary team.

Daily responsibilities will see you acting as a trusted assessor, performing triage and signposting individuals to relevant interventions and services. Due to the multi-agency nature of the service you will also be engaging with key stakeholders on a regular basis.

Moreover, you will get the opportunity to co-design and develop group intervention workshops to increase service user engagement. Working alongside clinical professionals to co-deliver clinical interventions.

You will be a strong team player, passionate about supporting people and willing to go the extra mile.
