We are looking for a remarkable highly motivated individual to join us as a Care Coordinator in our Rehabilitation and Recovery Service.

We are looking for a creative and empathetic individual to be responsible for working in partnership with members of the Recovery Centres network and city-wide Rehabilitation Pathway.

The Recovery centre is a multidisciplinary and multi-agency service made up of staff members from LYPFT, Community Links, Leeds Mind and Touchstone. The remit of the service is to provide coordinated care and interventions to service users within the inpatient rehabilitation and recovery units and to support their transition into the community.

As an experienced leader you will co-ordinate the care of service users from admission to the inpatient service, into the community and through the discharge.
