The Regional Pathway Development Service was formed in 2008, between Leeds Partnership NHS Foundation Trust (LYPFT) and Community Links.
The main purpose of the RPDS is to identify appropriate care pathways for clients leaving hospital environments, by offering a comprehensive review of their care and treatment plans and full assessment of their housing and resettlement needs.
Through the Yorkshire and Humber Personality Disorder Offender Pathway Housing and Resettlement Service a Housing and Resettlement Caseworker offers assessment and advice to Offender Managers to help facilitate safe transitions from custody to the community, working in partnership with the NPS, Psychologists and Approved Premises staff. Holistic and detailed housing assessment reports take a longer term view of housing and resettlement, utilising Psychological formulations.
The Housing and Resettlement Service is only available to clients who have been referred directly to the wider network of Personality Disorder Services. These referrals come via local CCG commissioners, forensic case managers or other relevant professionals, (in some services, with prior consent from local commissioners). In other areas of the network, clients are referred internally from the Care Coordination team or by offender managers.
