Bradford Dementia Hub is delivered by Community Links in partnership with Khidmat Centres and Bradford Bulls. It offers an information and advice service for anyone, of any age, living in Bradford concerned about their memory, living with dementia, and their carers, families and friends.
For more information or to make an online referral on behalf of yourself or a loved one, please visit the Bradford Dementia Hub website: https://bradforddementia.com/
Bradford Dementia Hub is commissioned and funded by Bradford Metropolitan District Council and Bradford District and Craven Trust (BDCT) Integrated Care Board. The service launched in July 2023 and follows the success of three other dementia services provided by Community Links: Calderdale Dementia Hub, Kirklees Dementia Hub and Young Dementia Leeds.
Offering an accessible and inclusive service is at our core. We aim to reach diverse communities including the South Asian, Indian, Middle Eastern, and Eastern European, ensuring our support is culturally appropriate and tailored to the individual’s support needs.
We offer information and advice throughout your dementia journey. We will support you whilst you wait for a diagnosis and guide you to the best support services that can meet your needs. We can also support you to identify what financial support is available to you and access services which can help with completing applications for this.
We offer one to one appointments with you and your loved ones. Appointments can be at a community session, on the phone, on at your home.
You can come to an information session held within the community.
