The primary role of the Recovery Centre is to support the successful transition and reintegration of service users into the community, supporting individuals to achieve positive mental health.
The Recovery Centre sits within the Rehabilitation & Recovery services in Leeds, based at the Asket site in Seacroft. Rehabilitation & Recovery Services are made up of the following components:
Community Links works in partnership with LYPFT and other 3rd sector organisations (Leeds Mind & Touchstone) to provide care coordination and support to clients through the in-patient R&R pathway.
Support is delivered via a multi-disciplinary team approach both within in-patient and community settings. The team consists of one Rehab & Recovery Lead, 6x Care co-ordinators (3xCommunity Links and 3x LYPFT), 5x Recovery/Peer Support Workers (1xLYPFT, 2xLeeds Mind, 2x Touchstone), 2x Consultant Psychiatrists, Social Workers, Psychologists, Healthy Living Advisor and a Dual Diagnosis practitioner.
The Recovery Centre operates within the hours of 8am – 8pm Monday to Friday.
We support male and female adults over 18 years who are leaving acute in-patient services. Potential referrals will be discussed with the service user and their care team prior to a referral being completed by their healthcare professional. Community referrals will be accepted and can be done via the service users LYPFT healthcare professional or via referral to the Single Point of Access (SPA).
