About the service

The Personality Disorder Accommodation Support Service provides housing and resettlement support to clients with a primary diagnosis of Personality Disorder.

The Leeds Personality Disorder Managed Clinical Network is a city wide multiagency and multidisciplinary service that aims to work effectively with people who present with personality disorder, complexity of need and significant risk issues. It provides a range of different services to meet the needs of people at the varying stages of their journey in the recovery process.

The Housing and Resettlement Service is only available to clients who have been referred directly to the wider network of Personality Disorder Services. These referrals come via local CCG commissioners, forensic case managers or other relevant professionals, (in some services, with prior consent from local commissioners). In other areas of the network, clients are referred internally from the Care Coordination team or by offender managers.

Support is delivered by:

  • Assessment of housing and resettlement need, based upon a psychological understanding of service user need.
  • Brokering of housing and resettlement packages from hospital to community based settings. Whilst we do not hold housing stock; resettlement pathways and services are identified through comprehensive mapping of housing and support throughout the region.
  • Consultation to locality based housing providers to support resettlement into the community, post hospital discharge or prison release within an identified period, within CPA and MAPPA frameworks.
  • Training to a range of professionals across inpatient, criminal justice and community settings, including both statutory and non-statutory services. As part of a wider Regional training programme, our team is also involved in the delivery of the Personality Disorder Knowledge and Understanding Framework training, commissioned by the Department of Health.

The service operates Monday – Friday 9am – 5pm.
