Community Options provides both 1:1 and group support to individuals with mental health issues in order to build skills and confidence to achieve their goals.
The service has two elements, a mental health community based service and self directed support provision for those who have personal budgets, direct payments or wish to pay for support privately.
We support people up to 2 years who are 16 years, under the care of a consultant Psychiatrist/PDCN, have a significant mental health need, are a resident in Leeds and are registered with a GP. The later provides support with a specialism around mental health issues for those age 19 and over, who qualify for a personal budget or who self fund.
Support is directed by the client and ranges from maintaining independence, help with daily skills, keeping in touch with relatives and the wider community, managing mental and physical health.
Together we develop a personalised support plan agreeing aims and tracking progress. You will be supported to access resources in your own community, engage in group work, including social groups and skills based workshops. get involved with the service, have a say in the activities and service developments.
If you have a personal budget you may have a support assistant and/or a support worker, this will be agreed with the social worker assessing your needs.
Community Options offer a seven day a week service, between the hours of 9am and 7pm.
