17 East Grange Drive is a supported living service for those ready to be discharged from acute care in Leeds. It comprises of four self-contained, fully furnished flats and is located in South Leeds. It is intended as a short-to-medium-stay accommodation service which has throughput to help relieve pressure on Leeds-based acute wards.
The Accommodation Gateway works with individuals being discharged from acute mental health wards, or those who are care coordinated under Community Mental Health Teams in Leeds, to ensure they are moved to accommodation which best meets their needs in a timely and effective way.

This service is run in partnership with Carers Leeds and provides a universal wellbeing support service to people in Leeds living with younger-onset dementia (under the age of 65), their families and carers.
