About the service

The West Yorkshire Fulfilling Lives: Supporting People with Multiple Needs Programme, known as West Yorkshire – Finding Independence (WY-FI) aims to improve the lives and wellbeing of people with the most entrenched multiple needs who are currently systematically disengaged or disconnected with services.

Aims of the service

The WY-FI Project is designed to deliver improvements to the service user journey for people who are:

  • Highly disadvantaged
  • In need of multiple and co-ordinated services
  • Currently, not engaging effectively with services or revolving in and out of services.

Improvements are delivered by enabling integrated working between services and agencies to achieve ‘system change’.

The project will support people who have a minimum of three of the defined needs:

  • Homelessness
  • Addiction
  • Re-offending behaviour
  • Mental health issues

And

  • Are not engaged with any existing services / project

The Kirklees Navigator team provide support to individuals in a variety of settings including within the community or at home. Navigators offer support to access and engage with services including support to attend appointments. Navigators work closely with a range of statutory and voluntary sector partners to support effective engagement with services.
