About the service

CHART Kirklees is an integrated substance misuse partnership led by Change Grow Live in partnership with Community Links, Locala and The Basement Recovery Project which supports people in Kirklees around their substance misuse.

Aims of the service

The Community Links Assertive Engagement Team (AET) delivers an outreach element to ensure that those who wish to engage with alcohol services, but are unable to access the mainstream service due to additional needs such as physical disabilities or anxiety, can still receive the same service in their own home.

The service is open to anyone in Kirklees, aged over 18 who would like to work around their alcohol use and require support to do so in their own home.

Support is tailored to the individual and the focus is around achieving their goals. This could include abstinence, planned reduction or harm minimisation.

The service operates: Monday, Wednesday, Thursday and Friday 9.30am – 5pm and Tuesdays 1pm – 7pm.

Appointments can be arranged outside of these times to meet individual need as agreed in advance with the service manager.


Please note that we do not accept self referrals.  If you are wanting support from the Assertive Engagement Team please discuss with your GP.
