About the service

The Dual Diagnosis team is part of the Kirklees Better Outcomes Partnership (KBOP), which brings the expertise of nine providers who support vulnerable people into their own accommodation, including those who are either homeless or at risk of becoming homeless. The aim of KBOP is to help develop the skills people need to sustain tenancies and live independently.

Our partnership brings together the expertise of all of our organisations, allowing us to support people with a range of complex social needs across Kirklees.

Some individuals may be fleeing domestic violence or struggling with mental health or substance misuse issues. The Dual Diagnosis Team offers a holistic intervention to people with mental health and substance misuse issues, focusing on harm reduction and reducing hospital admissions.

Individuals can refer themselves or be referred by a professional.

Aims of the service

We provide a high quality and flexible service for those who have found it difficult to engage with services in the past. We work jointly with both mental health and substance misuse services to provide a tailored package of support to everyone.

We use a range of psychosocial interventions, CBT and motivational interviewing techniques, as well as practical support and signposting for anyone aged over 16 and living in the Kirklees area.

Our ethos is to work in an open, honest and person-centred way, encouraging people from diverse backgrounds to make positive changes to their lives and take responsibility for their recovery.

We provide community-based support in the person’s own home or a local community location of their choice. We offer both face-to-face and telephone support for a wide range of issues.

The service operates Monday to Thursday 9am to 5pm and Friday 9am to 4.30pm.

For further information or to discuss our referral pathway you can contact us on 01924 448977.
