The West Yorkshire-Finding Independence (WY-FI) project is improving the lives and wellbeing of people with some of the most entrenched multiple needs across West Yorkshire. Beneficiaries of the project experience at least 3 of the following needs; homelessness, addiction, re-offending and/or mental ill-health. The project supports those individuals who don’t engage in services, revolve in and out of services or are excluded from services.
A Big Lottery Funded project, WY-FI is a partnership of 9 delivery partners led by DISC, working across all five local authorities in West Yorkshire to deliver improvements to the service user journey by enabling integrated and effective working between services and agencies.
As a Lead Navigator you will develop working relationships across your locality network of service providers, local forums, support groups and Commissioners; as well as provide support and challenge to your team.
