Overview of services
Community Links has a wide range of services and takes referrals from a variety of sources. People can oftern self-refer. Our services include:
Support in the home
The Community Links Mental Health Support Service for people 16 and over including older people in North East Leeds.
Supported accommodation Resettlement - Mental Health Housing support service for people with a range of mental health problems. The service is able to work with people for up to 2 years.
Sustainment - Mental Health Housing Support Service for people with complex needs. The service works with people on a longer term basis.
Short stay hostels
Two staffed, shared houses, one each in West and East Leeds offering a stepping stone between large hostel/hospital accommodation and independent living.
Three hostels providing 24 hour transitional support between hospital and permanent community accommodation, each also having respite facilities.
Longer stay hostels
Hostel accommodation with 24 hour support in Leeds with rooms in a shared house, individual bedsit flats and single self-contained accommodation.
A registered nursing/residential home in Leeds with 24 hour support for people with needs that exclude them from other mainstream services.
Personality Disorder
Accommodation Support (part of the Leeds Personality Disorder Clinical Network) is for people diagnosed with a personality disorder who need support in accessing or maintaining their accommodation.
Pathway Development Service (in partnership with Leeds Partnerships NHS Foundation Trust and Humber Mental Health Teaching NHS Trust) provides a smoother journey for people with a personality disorder diagnosis throughout Yorkshire and the Humber.
CL - Changes is a service in Kirklees and Calderdale which offers support to women with a personality disorder diagnosis who offend or are at risk of offending.
Early intervention
aspire is the Leeds service for people aged 14-35 with first episode psychosis.
Deaf and Mental Health Community Development project
The project aims to help all mental health services in Leeds to meet the needs of Deaf people.
Alcohol Support Service
CLASS is a service in Kirklees which offers support to people with alcohol problems.
Training
Mental Health First Aid (MHFA) is a team delivering training about Mental Health throughout Yorkshire and the Humber.
Dual Diagnosis service
The service provides an effective floating support scheme to individuals with moderate to severe mental health and co-existing substances misuse needs. It will be characterised by a strong ethos of partnership working at operational level.
Creative personalities
Creative Personalities is a collaborative project between service users, Community Links and Personality Plus, for service users living in the Yorkshire and Humber region.
On-TRAK
On-TRAK alcohol service for Kirklees, operating as a partnership between Community Links and Lifeline, the service is aimed mainly at moderately and severely dependent drinkers.
IAPT Community Links is part of the successful consortium which has won the contract to deliver phase 2 of Improving Access to Psychological Therapies in Leeds. The other partners in the consortium are NHS Community Healthcare, Touchstone, and Leeds Counselling.
Stop Hate UK and Community Links A unique national campaign starts crime reporting of mental health abuse. Hate crime affects 3 out of 4 people with a mental health problem.
Community Links Training Centre Situated in the leafy suburb of Oakwood (Leeds 8). Easily accessible by car or by public transport.This unique venue can be used in a variety of ways, e.g. AGM's, training, public meetings, mediation sessions etc.
