Community Links - Changes
- Community Links Changes supports women with complex needs who have offended or are at risk of offending, and covers the whole of Calderdale and Kirklees.
Community Links Changes works closely with Evolve, part of Calderdale Women’s Centre.
Service aims:
- Practical help, such as help with housing, finding occupational activity, parenting support
- Integrated support
- Improved capacity in other services to support the client group
- Reduce women’s offending and divert women from custody
- Reduce need for more intensive services
- Help clients to solve problems, form relationships, understand and manage emotions
The way we work:
- Full collaborative assessment
- Case management approach
- Brokering a package of multi-agency support
- Sharing good practice, including consultancy
- Working in a variety of settings including clients’ own homes, hostels or prison
- Using evidence based frameworks and tools
- Low caseload, short term concentrated support
Referrals
Referrals are from a variety of sources, including Evolve, probation services, police, and statutory and voluntary agencies. Clients must live in Calderdale or Kirklees.
